GeorgiaUnify facilitates closed-loop referrals and coordination of services such as housing, transportation, and nutrition for effective whole-person care throughout a community. The platform protects patient health information while creating a closed-loop referral system between providers and social care organizations.

GeorgiaUnify is vendor agnostic and can integrate with an existing project management system or provide that function for organizations without an electronic project management system.

GeorgiaUnify includes services matching, whole person needs identification and digital referrals. The platform helps users assign tasks to a coordinator and even fully automate routine tasks. Plus, the platform incorporates operational dashboards that establish performance goals and success criteria and then tracks progress towards those objectives.
 

The Georgia Medicaid Management Information System (GAMMIS) is connected to the Georgia Health Information Network (GaHIN). GaHIN’s clinical viewer within GAMMIS lets providers who service Medicaid patients search and access health information on individual patients.

For Georgia healthcare providers who may not have an electronic health record (EHR), the clinical viewer in GAMMIS offers access to patient information at the point of care using internet access. Also, it is useful for providers who may need to quickly view patient information without logging into their EHR.
