ATLANTA, GA | April 24, 2025 — The Georgia Health Information Network (GaHIN) has launched the GeorgiaUnify Resource Directory, a centralized tool that connects healthcare providers, care teams, and community organizations with essential social services across the state.

The directory helps users locate and refer individuals to non-clinical support services like food, housing, transportation, and behavioral health, ensuring more coordinated, person-centered care.

“The GeorgiaUnify Resource Directory helps break down barriers to care by connecting clinical providers with community resources,” said Denise Hines, Executive Director of GaHIN. “It’s a critical step toward delivering smarter, more compassionate care statewide.”

Available to GaHIN participants through secure login, the directory is part of GeorgiaUnify’s mission to better integrate health and social care. Learn more at: https://www.gahin.org/resources/georgiaunify-directory
